Policies & Privacy

In these challenging economic times, running a medical practice has changed dramatically.  Changes in insurance, added demands on a physician's time, and advances in medicine have all led to changes in the business of operating a medical practice. 

These changes have forced us to institute a set of policies in order to ensure that this practice remains economically viable so that we are here to serve you in the future.  Among the policies that impact you are:

Missed Appointments
 

Our time is valuable and often there are several patients trying to make appointments to see us.  While we realize that missing or forgetting an appointment is sometimes unavoidable we ask that you be considerate of others and let us know when you are not going to be able to make it to your appointment.  In the event that you do not notify us, it is our policy to charge a fee for the missed appointment.
 

Late Cancellations
 

Again, in order to help this office operate efficiently it is important that if you need to cancel an appointment that you let us know not less that 2 business days prior to the appointment.  If you have a Monday appointment we need to know about your cancellation by the close of business on Thursday.  This policy allows us to offer your appointment slot to someone else.  However if you cancel later than 2 days prior to the appointment it becomes very difficult to find someone to fill that slot and it ends up going to waste.  Thus it is our policy to charge a $25 fee for appointments cancelled with less than 2 business days notice.  If not paid when billed, this fee will be due at the time of your next office visit along with the co-payment required by your insurance.
 

Missed Surgical Procedures
 

In the event that you miss or late cancel an appointment for a surgical procedure there will be a $150 charge. 
 

Returned Checks
  We realize that at some time each of us makes a math error that results in a check being returned by the bank.  Unfortunately, our bank now charges us a fee for each deposit item that is returned for insufficient funds.  Therefore we must now charge a fee of $30 anytime a check is returned.
 
Insurance Co-payments
 

We realize that money can often be tight for all of us but it is now a matter of law in the State of Georgia that we must now collect your insurance co-payment at the time service is rendered.  In the event that you are unable to pay this fee at the time of your appointment we will be happy to reschedule your appointment to a time when you are able to make the payment. All insurance co-payments must be paid in cash.
 

Self-Pay Patients
 

New patients who do not have insurance are required to pay not less than the cost of the initial office visit ($175) at the time of the visit.  This fee is due at the time of service and cannot be billed.  Once established as a patient, self-pay patients will be required to pay $100 for each subsequent visit.
 

Referral Out
  If a patient misses two or more scheduled appointments we reserve the right to refer you out of our practice.
 

HIPAA Privacy Policy
